Destiny Family Medical Clinic is enrolled with most health insurance companies in our area. We accept major credit cards, including MasterCard, Visa, American Express and Discover. We also accept Medicare and Medicaid assignment. Insurance co-payments are expected at the time that the service is given.

For those without insurance, or with inadequate insurance, we can offer our Destiny Outreach program which provides a billing sliding scale based on your household income. If you desire to apply for our sliding scale, please call us or stop by our office. When you have completed your application you may mail or bring it to us along with the other documentation requested on the application. You must complete the application and return it with requested additional income verification information within 10 days of the date of service or the full amount of the visit will be billed to you. We will then notify you in writing when we have made a determination as to the percentage discount you qualify for.

The minimum up front charge for an office visit at Destiny, for those that are applying for a sliding scale discount, is $50 for the office visit (but it may be more depending on your initial estimate of household income before completing sliding scale information). Once the sliding scale discount is determined, then full payment of the discounted amount is expected at the time of each future service. For those without insurance but who do not qualify for a sliding scale discount, a 20% discount on the bill is offered if paid in full on the day of service.
