At Destiny Family Medical Clinic, we are blessed to have an amazing staff of highly qualified professionals, in addition to a clean, modern facility and advanced technology. While medical technology and expertise are powerful tools in fighting diseases, we understand that our clinc would not be possible without the grace and power of God, and to us, that's the most valuable medicine we can offer.

To better prepare for your appointment, we ask that you fill out all patient forms on our website (click on the sample form to the right) and bring them with you, along with insurance information and list of current medications.  Or you can choose to arrive 30 minutes early and our helpful receptionists can assist you with any questions you may have.  On most days, we can make same day appointments if you call between 8:00 and 9:00am.

To Our Patients
Thank you for choosing Destiny Family Medical Clinic as the health care provider for you and your family.  We have included this informaiton to explain how our office operates.

Office Hours:  Destiny Clinic is open Monday-Friday 9am to 5pm.  Our office number is 605-339-3378.  If you should have an urgent issue arise that cannot wait until the next clinic day you may reach the doctor on call through the Destiny after hours on call number which is 605-339-3377.  If you have a life-threatening emergency go directly to the nearest hospital emergency room.

Appointments:  Destiny offers both same day appointments and advance appointments.  When scheduling an appointment with our office please explain the reason(s) for your visit.  This will allow us to schedule the appropriate amount of time needed to address your helathcare needs.  Please bring your insurance card, co-pay and list of your current medications with you on each visit.  Walk-in visits for acute illnesses or injuries are accepted if you are willing to wait and there is time in the schedule.  Scheduled patients will always be seen first except in the case of emergenices.  If you find that you are unable to keep your appointment we do request that you notify our office as soon as possible, but no later than 24 hours prior to your appointment.  This will allow us to utilize the time reserved for you for another patient who may be ill.  It is our policy that if a patient misses 3 appointments within a 6 month time period without providing 24 hour prior notification to us they will be seen on a walk-in basis thereafter for a period of 6 months.

*Please inform our front desk staff of any change in name, marital status, address, telephone number, insurance coverage or employment. 

*Please inform the nurse of any change in medications or medical history.

Medical Test Results:

Please allow up to 5 working days for test completion and notification. For lab, x-ray or any other test results, the provider or nurse will either call you or mail you a letter with those results as soon as they are available, or you will be scheduled for a return appointment to go over your results.

 

Prescription Refills:

Please evaluate your medication supply prior to your office visits and request your healthcare provider to write your refill prescriptions at the time of your visit. The general practice of our medical provider staff is to prescribe medications along with the number of refills that would last until the provider feels an office visit would be important for follow-up of the condition that the medication is prescribed to treat. If your provider has authorized refills of your medication, please call your pharmacy when you have approximately 1 week left on your current supply.   The pharmacy will then send our clinic a fax request for refills. Please plan on it taking 48 hours or more from the time you request the medication from the pharmacy until it is filled.

 

We do not refill any controlled substances (pain medication, anxiety medication) prescribed by another physician without seeing you first in the office. Nor do we call in prescriptions for illnesses for which you have not been evaluated.   An appointment is necessary to ensure you receive the appropriate treatment course for your current illness.  If you utilize a mail in pharmacy we will write the prescription, but it becomes your responsibility to mail it in.

 

Medical Records:  Your medical records are strictly confidential. The Health Information Portability and Accountability Act (HIPAA) restricts us from releasing any information without your written permission. There may be times when you may request that we provide copies of our records on you to other entities. We do incur an expense to provide you with this service and that cost may be passed on to you if the cost for the copies is not reimbursed by the receiving entity that you have authorized to obtain these records.

 

Questions:  We have an excellent staff that is well trained in assisting patients. They are available to assist you with any concerns. For medical questions please give a detailed message to a member of our qualified staff and a phone number where you can be reached. The nurse will return your call as soon as she has the opportunity to talk to your healthcare provider about your questions or concerns.

 

General Billing questions:  Please call our billing company, DT-Trak at 877-892-5784, ext. 223, and speak with them directly.  If unable to get your questions answered, then feel free to call our office manager at the clinic. Our staff is here to help you.

 

Lab Bill Questions:  Please call Lab Corp directly at 800-343-4407, if you have questions concerning your lab bill.

 

Insurance and Payment for Services:  We are contracted with a large number of insurance plans. As a courtesy we will file your insurance claim for you. It is our policy to collect from you all insurance co-pays and preferably also all deductibles at the time of your visit. Accurate insurance information is vital for processing your claims. Please provide any new information regarding your insurance to the front desk. Please remember that insurance is considered a method of reimbursing the member for fees paid to the doctor and is not a substitute for payment. Payment for the account remains your responsibility. Payment is expected at the time of service. For those wanting to pay for their visit in cash, we offer a 20% discount if paid in full at the time of service. We accept cash, checks, debit cards, and MasterCard and Visa credit cards. We strive to have a mutually respectful relationship with all of our patients. As part of that relationship we believe it is important that you maintain your account with our office in good standing.

 

Discount Care Program:  We also maintain a Discount Care option of payment for patients without insurance. The patient must complete a Discount Care application, and supply the required supporting documentation to determine if the patient qualifies for this program. The cost of a visit under the Discount Care Program is $50 for the consultation with the provider, plus any additional costs related to testing and procedures. These additional costs are usually discounted as well. The $50 fee generally is for a standard 15 minute office visit, and collected prior to seeing the doctor. Any additional costs will be explained to and received from the patient prior to performing the testing or procedures. An agreement that Destiny has with LabCorp, a national lab, allows us to give substantial lab cost discounts if paid for at time of service.
